Online triage (AccuRX)

Patient Online Triage (Total Triage)

Clifton Medical Centre uses a Patient Online Triage (Total Triage) system to manage both medical and administrative requests. This system helps ensure that patients who most need medical care are prioritised and that requests are dealt with by the most appropriate member of the practice team.

When you contact the practice, you will be asked to complete a short triage form. This allows a GP or senior clinician to review medical requests, and the appropriate member of the team to review administrative requests.

Depending on your needs, you may:

  • Receive medical advice

  • Be contacted by phone by a clinician

  • Be sent a link to self-book an appointment

  • Be booked directly into an appointment

  • Receive help with an administrative request

  • Be directed to another appropriate service or member of our clinical team


How to Contact the Practice

You can submit medical or administrative queries by:

  • Completing an online triage form (Accurx)
  • Attending the surgery in person
  • Telephoning the practice

If you attend the surgery or call the practice to request a GP appointment, a triage form will still need to be completed.

Our Reception Team can help complete and submit the form on your behalf if necessary.

Practice opening hours for enquiries are:

Clifton Medical Centre
8:00am – 6:30pm

Branch Surgery – Wickersley
8:00am – 5:00pm


Accessing the Online Triage Form

The Accurx triage form can be accessed via:

  • The NHS App

  • The Clifton Medical Centre website

If you are struggling to access or complete the form, please contact the practice and a member of our team will be happy to help.


What Happens After You Submit a Request?

Once your request has been submitted, it will be reviewed by the appropriate member of the practice team.

  • Medical requests will be reviewed by a GP or senior clinician.

  • Administrative queries will be reviewed by a member of our Administrative Team.

After review, you may:

  • Receive advice by text message or phone

  • Be asked for further information or photographs

  • Be sent a link to book an appointment

  • Be contacted by the practice to arrange an appointment

  • Receive a response to your administrative request

All requests are assessed based on clinical need and urgency.


Providing Information on the Form

When completing the triage form, please include as much information as possible about your symptoms or request.

Providing detailed information helps our clinicians assess your request more effectively and ensure you receive the most appropriate care.

If there is not enough information in your request, you may be asked to resubmit the form with more detailed information.


If You Cannot Complete the Form Online

If you do not have internet access or find forms difficult to complete, please contact the practice. Our Reception Team can complete the form with you over the phone or in person.
